In addition to featuring the fourth Quarterfinal match in the Lightweight Grand Prix between Shinya Aoki and Katsuhiko Nagata, which Aoki won via Gogoplata submission, Dream 4 featured all four of the Quarterfinal bouts from the Middleweight Grand Prix. In tournament action, Gegard Mousasi, Zelg Galesic, Ronaldo Souza, and Melvin Manhoef advanced to the tournament Semifinals.
After winning a qualifying bout at Dream 3, Manhoef replaced the injured Kiyoshi Tamira in the Quarterfinals at Dream 4, brutally knocking out Kazushi Sakuraba to earn his place in the Semifinals.
