1. I didn't take anything, I'm innocent! (I have no idea why it showed on the test.)
2. My doctor/pharmacist/coach/sparring partner/uncle/neighbour gave me something and never told me there was anything banned in it! (I never checked what I was taking because reasons.)

I was ill/injured and took something for that, apparently it contained banned substances too. I plead stupidity, not guilt.

Weidman and Aldo keep having to drop out because of "injuries".
Jon Jones suddenly has 1/4 the testosterone of a regular male.
GSP starts pushing for stricter testing as soon as he is accused, shrinks visibly and looks a lot weaker in his next fight, and then retires.
The examples go on and on. And to be honest, if one of those guys pisses hot I won't be disappointed, nor will I say it detracts from their accomplishments. If everyone is on it, no one is (competition-wise. I know some of the nasty long-term effects). My jiu-jitsu teacher told me one year ago that if someone is highly ranked in the UFC, that person is on something. And it looks more true with each fight.
EDIT: Same thing with no-gi grappling and Pride.

Whatever is being done to combat steroids in MMA it has clearly not been working. And does the UFC even want the sport to be clean? Jack Slack had it right: "There's a reason the UFC stopped additional testing. Higher costs, more injury drop outs. Paying to ruin your own fights"
